Summary
Mental Health Intensive Case Management (MHICM) is CARF accredited program that fully embrace and incorporate the core principles of psychosocial rehabilitation and recovery in all interactions, interventions, and program development.
Duties
The primary responsibilities of this position are to provide intensive case management services, continuous quality improvement activities, evaluation/consultation, psychosocial assessments, patient groups, individual therapy, placement services, and referrals to various community agencies and hospital services to MHICM clients. The senior social worker in MHICM functions at a fully autonomous level. While working in the community, there is no immediate supervision or ready access to consultation. The senior social worker must appropriately assess, plan, and implement immediate response in situations that may have the veteran or the social worker at risk of harm. The senior social worker will assess need for hospitalization, and frequently will complete admission documentation. The clinician may need to assess for and complete a legal hold. Specialized knowledge of practice related to mental health recovery and the complex discharge planning required. Services in serious, complex, and complicated cases, often referred by other social workers and interdisciplinary team members. Independently performs assessment of patients for psychosocial needs. Incorporates complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved scope of practice. Performs specialized treatment of complex physical and/or mental illness. Requirements
